I have permission to share this......Some of you may know that my daughter was in the ICU on a vent for the first week of my granddaughter's life, had a massive hemorrhage and also has breast implants...and my grand daughter has never had formula. (She was exclusively breastfed until starting solids and she's now 2 and still occasionally nursing).

We had donor milk for the first week, started pumping at less than 24 horus but didn't get an appreciable amount of milk until about 12 hours after she was extubated. She eventually got a great supply; enough to donate back to the milk bank.  She used a Freestyle pump at work, and had my Lactina athome. She did need to do some power pumping in the first two weeks, used Fenugreek briefly and worked to get a good milk supply established but I think that was more related to her medical experience than the implants.

Laurie's note about flange size might be on track as well as the other suggestions about the pump. I find that it is extra important to avoid engorgement with implants to avoid damage to the alveoli as well as FIL from an overfull breast.
I'm not aware of any research on supply and the influence of implants...maybe I should write a case study for Clinical Lactation about our experience?
