Pat,
I recognize that this mom denies she is under stress but I can't help but
believe that this maybe the problem.  Many years ago, I was under the care of
a holistic health care professional, who asked me because of my health
problem whether I was stressed.  I told her no but years later I recognized
that I was severely stressed.  The reason I said no was partly because I
didn't really know what she meant by stressed and partly because I had become
so use to the emotional stress level in my life that I considered it normal.
It was only in retrospect that I could see that I was under incredible
amounts of emotional stress and my body was responding to that stress.  On
the outside, I appeared calm and relaxed but if my blood pressure was taken
it would be incredible high.

Niles & Michael Newton did some very interesting studies on the let down
reflex.  They said, "Emotional disturbances, embarrassment, and pain inhibit
let-down to the sucking baby, and thus the baby gets little milk."  I would
suggest to this mother that she record the sounds and events that are going
on when she breastfeeds this baby particularly when it seems that the reflex
seems delayed.  She may not think she is stressed but a mother with 4
children and her lactation history may very well be stressed about feeding
this baby.  Her saying she is not stressed and appearing calm does not in my
book mean that she isn't stressed.  Just a thought...
