It can go either way, too big or too small.  The majority of women who need a 
different size need a larger flange.  Way more moms need a larger Medela or 
Ameda flange than those who need Medela's 21 mm flange or Ameda's insert.  
So, naturally, most of the mothers who have problems are using a flange that 
is too small.  Condensation/backup can happen if they are using a flange that 
is too big, but I don't see that as often.
