So if you use a R... Teddy Bear card then we know "confidentially" that you
are using formula?  I'd design a generic card and have hosp print shop
print them up  on 3 x 5 cards.  We used to denote BF by highlighting lower
left hand corner with a small block of yellow, so just by glancing at card
you knew br or bot.  Funny story, father stood at viewing window and must
have been surveying all  the cards.  Came to nursery door and said" my baby
is jaundiced too, why isn't his card highlighted with yellow?"
  P.S. yes, I think legal is pushing.  So just do the generic bit and drop
