Has anyone worked with a mom with MTFR syndrome? Moms have trouble with
bleeding, multiple miscarriages. My patient required 5 mg of folic acid
daily to remain pregnant (after 5 miscarriages). She is still taking
that plus prenatals. Baby was born with very misshapen head, extremely
poor suck, what I suspect is velopharangeal insufficiency, arches and
fists. Mom has marked breast assymetry, large undefined nipples. I
first saw baby at 6 weeks. Baby was on the 5% of the WHO weight charts.
Baby took 6 ml at breast and mom pumped 40 ml. Baby could not suck on
Haberman, breast, bottle, etc. I recommended CST, pumping and herbs for
mom, suckle-training and supplementing with the Haberman. Today I saw
baby and he is gaining along the curve. Mom is still supplementing but
baby is getting almost all breast milk (from almost all formula). Baby
still "snuffles' when sucking but is able to latch to mom and create
some suction when flexed and using large nipple shield. We tried
putting baby prone over the arm encouraging flexion and more suction in
that position. Mom is now up to making about 24 ounces per day. Mom is
doing fenugreek and domperidone (started the latter about a week ago).
Pediatrician doesn't want to see baby until 4 months (baby is 11 weeks
now). I had encouraged mom to wear baby in sling but she stated baby
would stiffen out legs and hated the sling. Today I showed her how to
"buddha pose" him in the sling facing her, which he accepted, relaxed
and finally was able to suck on the Soothie pacifier. Mom wants to know
if the MTFR syndrome can affect supply. I did see some reference about
gastroparesis and MTFR, Parkinson's disease etc. This is a very
dedicated mom who nursed her first baby 4 years! I believe that her
supply problems are really caused by the baby's poor suck and she's just
running to catch the freight train that already left the station!
Anyone with knowledge on this condition, please jump in.
