 I like to start with the simplest things first. In this situation i would have wondered if the mother had extroverted nipples that got pushed in with a tight or new bra causing a ring of moisture that never dried out. Simple irritation. ointments, etc. just continue to hold the moisture in especially if she didn't dry her nipples well before closing her bra.
