Barbara's post reminded me of something I came across years ago.  Barbara
suggests that mom try leaning back into a pillow to raise her breast off the bed
a bit in case the side of the breast is getting "smushed."  There is another
possibility here, related to positioning.  Sometimes mom and baby start out
facing each other in the side-lying position.  But then both fall asleep before
the feeding is finished.  While asleep, mom and baby sometimes each tend to
roll a bit backward, but baby does not let go of the nipple.  So mom's nipple
becomes stretched.  This can lead to nipple strain, which can cause severe
shooting pains through the breast.  Sometimes these pains only happen at night, and
sometimes they continue to happen through the day.  If, upon questioning the
mom, this seems like it might be the problem, suggest that mom place a pillow
directly behind herself and behind the baby, so that they can't roll away from
each other in their sleep.
