Hello wise ones!
I am working in a breastfeeding clinic under supervision of a  
CNM/IBCLC. We have become the local source for domperidone  
prescriptions, so at this time have several patients who have been  
using the drug long term. We typically try to get the mom to start  
weaning from the medication once their milk supply has stabilized, but  
many of our moms notice a decrease in supply once the dosage is  
reduced (psychological? Stress related? physiological? who can  
know...), and so they want to stay on the meds long term. Our typical  
dose is 30mg three times/day. (I know that Hale recommends 10-20mg  
3/day, while Jack Newman recommends 20mg 4/day, so I think we're about  
in the right ballpark) Anyway - what is the opinion of this group?  
Should we be more aggressive in recommending these moms work at  
reducing the dosage? Is there any known risk to being on this dose  
long term (as long as a year)?? Any input is appreciated. I know the  
drug at these doses is really under-evaluated, but if anyone knows of  
any published research, that would also be much appreciated.
