If obvious causes are ruled out, it is possible for a woman to have
'idiopathic galactorrhea,' as mentioned in this webpage:

IDIOPATHIC CAUSES

Idiopathic galactorrhea is a diagnosis of exclusion. Galactorrhea is
considered idiopathic if no cause is found after a thorough history,
physical examination, and laboratory evaluation. The patient's breast tissue
may have increased sensitivity to normal circulating prolactin levels.
