Here's a lactnet comment from Ros Escott (Australia) some years ago, after
she visited a Baby-Friendly Thai hospital where the "Three Sucklings"
(early, frequent - with no separation, and correct) are observed:

> No separation means that the mother stays with the baby for everything.
>Frequent suckling means that the mother lies in bed on her side with her baby
>at the breast.  If you asked a mother how many times per day she feeds, she
>would look at you strangely.  Better to ask whether the baby ever falls into a
>deep sleep and drops off the breast for a while - they occasionally do.

I've never worked in a hospital and have little first-hand experience with
the first two days.  But with the relatively small amounts of colostrum that
are available then, it makes sense to me that there wouldn't be continuous
swallowing for 6 hours, that some babies would find deep solace in
near-continuous feeding, and that output would be fine given even occasional
swallowing during those extended feeds.  Once her supply builds, he'll
*have* to fall off... or he'll explode.
