Jan, this is so true even here in Pakistan.
On arrival to LW, the mum is given an enema (-she might soil the accouchers
hands!) bugger the possibility of prolapsed cords(high heads and all) and
other problems as she strains!
Next, she is canulated(an access line for drugs she needs!)- and to make her
uterus contract properly(nature IS slow!) she has Syntocinon drip started
10units for Primis and 5units in Dextrose 5%in water.
 These ladies are not encouraged to drink fluids, in case of LSCS's.(nature
doesn't provide high energy solutions, so it's dextrose 5% in water!)
Next, they are confined to the delivery bed, flat on the back,(bugger
compromising the baby and the mothers circulation and 'unnatural
gravitational challenge'!). I find a devil of a job 'to convince mums' 'to
move onto their sides'- 'let alone get up and walk or go to the toilet'- no!
convinced 'they are sick'(having a baby!), they want a pan for the
elimination of urine,- and feaces often after (the enema).
All primiparas must have a epissy cut, often 'to soon' for my liking- 'cause
the head pressure hasn't lowered the blood supply, so copius bleeding till
that happens is common. If the multi appears to have a tight peri????? it is
cut too!- mind you the head hasn't even reached it, to show how tight it is
it's the midwives fingers stretching it and yelling 'Push, push' and the
lady red in the face, abliging the order, as the last VE said fully so
push------ (bugger! waiting for transition and natural pushing urges to
occur!(NATURE IS SLOW)
Next, the bub's head and body are delivered 'in a rush(often jet propulsion)
and the baby is suctioned with a bulb sucker, down to it's toes, flat on
it's back!( instead of raising it onto mum's abdo and allowing mum to greet
and dry it while it naturally resuscitated with gentle caresses from mum and
staff still attached by the pulsating cord and as head is lower than body
drain the lung fluid naturally.)
But NO, they can't have this happening, the cord is cut and  bub whisked to
the resus trolley, where it is suctioned and oxygenated until it glows pink-
why---OR given Sodi Bicarb and Glucose 25% equal parts into the cord and
some adrenaline too!- cause they had to get it going!!(Natures course-
NO! -medicallized arrival).
 Next, the Placenta within 10minutes of birth or they remove it manually-
the body can't be allowed to shut this organ that has supported life for
9months sucessfully!!!it's self. With this happening the body is injected
with more Syntocinon and Methergin(Ergot) IV to prevent PPHs and she has
peri sutured and midwife departs. No giving bub back to mum it is left on
the resus trolley and the ayahs and aids clean up wash and dress bub and
give it to relatives- the delivery bill is paid and Mum, bub looking like a
small parcel carried by Ma-in Law and the retinue depart the hospital.
What do I hear you say- Mum feed why no the staff seem to support ma-in laws
idea there is no milk in the breasts or it is poison for the first three
days!! ------whatever the excuse!
Since I have been here (almost 12mth) I am gradually turning some of this
around with the deliveries I am in on!
Mums get up and walk around as long as I can get them to stay up!
Bubs go to mum and are helped to attach- Most mums appear not to have the
slightest idea about the babies need even those who have other kids as Ma-in
law seems all powerful and dictates! I sneek the bubs to the breast before
she gets her hands on it so it 'is' feeding when she arrives!
No, she doesn't get to take it of the breast, as she won't touch it, cause
it's still dirty!!!!
