Here's a drop of colostrum for this discussion.

While Lee feels that Medela deliberately makes its promotional flanges a bit
uncomfortable to force a mother to buy more product, Susan says this doesn't
make sense.

My view of this is that the standard flanges that come with the basic pump
kits are a particular size that will work for women with 24 mm nipples, but
won't work if the mother has nipples that are larger or smaller. Nowadays,
the flanges come with a base that can be used with different sized flanges.
This is an improvement over the 90s, when the whole flange set-up was one
piece and not two.
