Dear friends,

I was contacted by a very good second stage NICU doctors (worth their
weight in gold). They have a great results in supporting breastfeeding in
their very preterm patients, as far as I know nearly 90% discharged home
breastfeeding from their department, and KMC is really implemented there.
So I really want to help them
They are asked me to help them collect evidence, current protocols to
clarify the issue with CMV infection prevention in preterm babies.
In their clinic (actually  the main children hospital of Ukraine)
breastmilk testing by PCR was started in all mothers whose children are
admitted to intensive care unit.
 If that milk PCR test is positive - even when mother  has negative test of
blood and urine and she is clinically healthy and no signs of CMV infection
- she has to stop provide her milk to the baby. When that healthy mothers
with the babies are transferred to the second level of care they often
stopped breastfeeding or just do some milk expression and not feeding it to
the babies.
Those great team that contacted me want to help those mothers breastfeed
and to make the practices in their clinic (and in the country as this is
the main clinic and their practices will be spread to other NICUs) really
evidence based (so they need good evidence!). Is this routine milk testing
done anywhere?
There is no protocol now as I know and nobody is able to provide references
for this milk testing.
Current protocols (and also protocols for milk banks donor testing) will be
greatly appreciated as well as other suggestions. I promiced that i will
ask the best experts in breastfeeding in the world


I forund the Nutrition toolkit but it was last time revised in 2008.
