Nola--have you tried a nipple shield on that side?  I would try having the
nipple shield ready and folded back, pump to pull out the nipple (or have
mom twiddle it out with her fingers), keep it pinched out with one hand,
put the nipple shield on with the other, and hopefully the nipple shield
would suck the nipple in and keep it out.  In terms of folding the nipple
shield back--make sure you turn the soft part as well as about half the
teat part itself inside-out.  Then when you place it on the nipple and
unfold it, the nipple gets sucked right inside, ready to go.  (I had read
about folding it back a while ago but wasn't doing it right until
recently--I didn't realize you had to fold part of the teat back also to
get this to work--it's really nifty how much farther the nipple gets inside
it.)
