Thanks to Evelyn for the encouragement to post this! I tweaked a little for
more context.

The symptoms you describe sound a lot like a problem I had.  I called my
doctor, and was "reassured" that babies absolutely cannot have problems with
lactose unless they have the very rare condition of congenital lactose
intolerance, which would produce far worse symptoms.  Fortunately, I decided
I didn't believe the doctor or the second opinion I received and did my own
research (I will admit that their advice was better than "try formula",

What worked for me:

Drastically reducing the sugar in my diet showed results in both of my
children (normal, non-green stools and no colic) in 12 to 24 hours.  I
re-introduced and reduced sugar several times, with consistent reactions in
my children.  This worked because lactose is milk sugar, so reducing
maternal sugar intake sometimes reduces the amount of lactose she can
produce.  Reducing dairy intake will not affect lactose (as I understand
it), although I hear it may make a difference if the problem is actually a
dairy / casein protein allergy in the baby.  Since I had been drinking about
a half-gallon a day of fruit juice to stay hydrated, sugar was likely a
significant factor.

Eventually, the symptoms returned, but this time clearly accompanied by
over-active letdown and oversupply, so probably was foremilk / hindmilk
imbalance.  Reducing sugar again for a couple of days provided a temporary
fix.  Block nursing solved the whole collection of problems permanently, and
I resumed my normal sugar intake without problems once my supply decreased.
