I believe that breastmilk is not sterile.  Sterility implies freedom from
living microorganisms.  Breastmilk is a living substance--white blood.  Through
the birthing process an infant encounters bacteria.  Bifidobacteria, becomes
the predominant bacteria in a breastfed baby's gut, because it is stimulated by
the oligosacchrides and nucleotides of human milk.  The formula-fed infant has
little or no bifidobacterium because infant formula is devoid of these
components or the synthetic/novel oligosacchrides and nucleotides do not work like
the real thing.
