I just attended a talk today by Paula Meier and she mentioned the study.
She commented that with the lactoengineering they do at Rush, they routinely
adjust the fat and calorie content for their NICU babies, so that each baby
gets what they need to grow.  I hope I am correctly representing what she
said but what I understood her to say was that if they have a baby who is
not gaining adequately on human milk, they investigate why and how to
lactoengineer so that the baby will gain.  They do not just default back to
formula.  It's all about the science of human milk, in her words, and
finding solutions that allow the baby to be maintained on human milk, not
defaulting to formula.  She was, as always, very impressive and very
eloquent on approaching the care of NICU babies from a perspective of what
needs to be done to get the baby to grow well on human milk.  So, I believe
it would be safe to say that SHE (and likely others, it's not a huge leap)
anticipates that some preterm babies won't get the calories they need in the
volume of milk they are able to consume on normally pumped milk (meaning
milk that hasn't been lactoengineered to have a higher fat/calorie content),
for a variety of reasons.
