Dear Wise Ones,
Maybe someone can shed some light on this case of mine.  I have a client
who is 24, just had her 1st baby.  Breasts are largish, and I saw her on
day 7 and they were hard towards the back of the breast but soft towards
the nipple.  Baby was always screaming, mother had an exhasusting labor (
she stayed at 9 cm for about 14 hours) .  Birth was without medication,
baby was born vaccum.  Baby was never settled.  Checked latch, etc which
was fine, but there were very few swallows.  I asked mother if back of
breast was always hard like that, she said no.  However, she reported to
feel no difference pre and post feed.
We started her pumping with a Lactina, and she started giving formula
supplements.  She pumped every 2-3 hours and was getting 5-10 cc each time
for about a week.  She then switched to a Avent hand pump, and has been
getting about 30 on one side, and still 5 on the other.  Same pumping
stimulation.
I asked her again maybe something she left out in History... Thyroid,
Placental fragments, (she is not bleeding anymore).  But she did say when
she was about 12 she ran into a bannister and severely banged that breast
(the same one that is barely producing) and was in major pain for at least
a week.  Can this be related to the small output?  Why is her other side
not producing more?  Why was the Lactina not working?  Has anyone ever seen
anything like this?
