"I now have concerns about even suggesting moms get a second opinion... This contradicts the information given by the doctor."

We are not bound by IBLCE's attempted Scope of Practice. It doesn't appear on their websites, and we old-timers haven't received a copy with instructions about implementing it. For the moment, at least, it doesn't exist. What *does* exist is IBLCE's 2004 Code of Ethics, which reads, in part, that the IBCLC must:

4. Conduct herself/himself with honesty, integrity, and fairness.
7. Base her/his practice on scientific principles, current research, and information.

11. Provide sufficient information to enable clients to make informed decisions.
18. Present substantiated information and interpret controversial information without personal bias, recognizing that legitimate differences of opinion exist.

There are tactful and tactless, graceful and graceless ways for us to provide information that the mother has never heard before. Telling her outright that her caregiver is an idiot, in essence, telling her that she was an idiot to choose him, and no empathetic LC would consider approaching it that way. But at the moment we're bound *to* provide accurate information if his isn't based on scientific principles and current research. There are lots and lots of good, gentle, empathetic, uplifting ways to do that, and we've all been doing it like that for years, ideally even enlightening the caregiver in good, gentle, empathetic, uplifting ways in the process.

We're not *allowed* to base our practices on misinformation. Let's all remember that simple and sensible guideline while IBLCE sorts itself out.
