Hi All,
Bonnie, I have direct experience with this situation myself and though I
cannot offer you all of the info you are seeking (how common is it...) I can
offer one bit of advice to the moms.

One way to help ensure that the nipple stays at the juncture of the
soft/hard palate is to put pressure at the underside of the breast (while
baby is latched on) just in front of baby's bottom lip.  For myself and
other women I have worked with with this very same problem, it has kept the
nipple from slipping from the junture into the bubble.  Of course L-O is
critical and they must maintain proper positioning at all times.  SUpporting
the breast is important, too.  Suck training (getting the baby used to
having something back at the proper juncture) can be especially helpful as
many of the babies I have worked with (my own included) have very sensitive
gag reflexes, and have already become used to stuffing the nipple into the
bubble palate rather than taking it far back into the mouth as is proper.
Onve they get used to the idea that something is supposed to be back at that
juncture, they seem to forget about stuffing the nipple into the bubble, but
mom must take care to not allow the nipple to slip back there.  Putting the
pressure at the bottom of the breast just at the babys bottom lip seems to
really help with this.
