If the mother in question has been having good prenatal care I would defer
to her physician for thyroid monitoring and management. If all signs were
good or reassuring with the way breastfeeding was going on day 6 I would
just recommend optimal breastfeeding practices and management. In light of
her history of low milk production I would recommend more frequent
monitoring of her baby's weight gain and other indicators of well being. If
there were any signs of less than optimal breastfeeding on day 6 I would
recommend deep latch, frequent feeding with breast compression, and Post
Feed expressing the amount of that would depend on how far from optimal the
breastfeeding was. I err on the side of overcompensating.
