Donna,
You didnt say how much intervention this baby had in the NICU. Was he
intubated? agressively suctioned? that plus the agressiveness of the mother's
"excercises" The baby is orally defensive. You need to contact the OT with
your observations of the mother's skills (or lack thereof) and have the OT
re-evaluate. The treatment MUST change to provide more positive oral
contacts.
Mom may be 43 but she sounds a bit immature. Was she infertile because of
age? (like she waited to long to try?) or had she been trying for 10 or 15
years without success?
Try mothering her a bit, friendly calls, modeling of how to interact with her
baby. (You did it by  talking to him,  cuddling him and praising the baby to
show her how. >>I played and talked and sang, and gently asked to come in his
mouth, he opened and let me assess.<< ) Very often mother's of premies will
say that they feel like they didn't even have a baby and you need to continue
help her with the bonding. Show her how to *gentle the baby to the breast*,
just leave him there but don't ask him to nurse. Get some good skin to skin
contact going and hopefully the mothering hormones will take over.
