         I concur with Nina's recommendation to do a swab. I work in 
a community with a large aboriginal population (North American Indian 
in your terms)

We had a recent inservice advising us to keep on the watch for 
"interesting" or "odd" infections.

ca MRSA (community acquired methacillin resistant staph A) is 
becoming endemic in North American Indian populations.

This may, of course, just be a run of the mill infection, but it 
should be tested.
The source is unlikely to the breast or the milk, more likely to be the child.

Is is possible to have a nasal swab taken of the child (and the mother)?If
memory serves, impetigo is either group A strep or staph aureus infection
that is commonly carried in the nose and throat.  Recurrent infection
indicates the need for systemic treament..
