I'm adding belatedly to this thread that when a mother I worked with
reported this situation she had been feeding a formula with added rice
as a thickener.  Her baby was prone to spitting up, but healthy.

What happened after you talked with her?  Did she stop using the thickened formula after you talked with her, and if so, was there a change?  Or do you know?
