I would respectfully inform the mom that pumping is hard on breast tissue.
Breasts are not "meant" to put up with the yanking for months.  So she may
want to consider nursing her next baby to avoid this pain.

For now, I would try "pumpin pals" angled flanges.  If she used to be comfy
using the 24mm, I would try the med or large...if she is using 30mm, the
extra-large.  And use the lowest amount of suction that still gets enough
milk.  And a heating pad on "low" for 5 mins after pumping, over the
clothes, for probable vasospasm.
