how old is this little guy now?
 
is he uncircumcised?  (if so, the pre-cath foreskin  retraction could still 
be causing some issues?)
 
during the "screaming when he urinated" episode, any signs of  dehydration?
 
where is he on the growth chart for ht and wt?
 
has there been a second UA to check ketones?
 
what was the Specific Gravity of that first and any subsequent UAs?
 
how often/long does he BF?
 
has he lost any weight recently?
 
Suggestions: 
 
-if must give solids, or to facilitate BM, give him  some fresh, cooked and 
pureed *dark green* veggie, ie: green peas, spinach,  green beans
 
-also to facilitate BM: will he drink a little diluted prune juice?
 
-in addition to what you get here on Lactnet, find a local LC or another  
experienced LLLL *pair of eyes* to take an objective look and help you  sort 
this out
 
-delay concerns about the "window of opportunity" issue
