Jacqui posts with a query about hydronephrosis diagnosed in fetal life by
ultrasound and possible treatment scenarios.
I've never heard of a baby being put on prophylactic antibiotics for this.
Never.  Certainly not without a single episode of infection.  
Artificial feeding is a risk factor for urinary tract infections, at least
in girls.  The gut flora of an exclusively breastfed baby is dominated by
lactobacilli, whereas escherichia coli, the most common pathogen for urinary
tract infections, are more abundant in the gut of an artificially fed one.
Could it be that the pediatric specialists who work with your doctor have
never seen an exclusively breastfed baby with hydronephrosis?
We used to get someone in about once a month with suspicious findings on
prenatal ultrasound, indicating possible 'cystic kidneys'.  I realize
hydronephrosis and cystic kidneys are not necessarily the same animal.
Maybe the ultrasound machines are better or the people doing the scans are
more knowledgeable, because it's been ages since I saw one of these.  These
babies were always referred for ultrasound of their entire urinary tracts
and I can only recall one in about ten years who actually had a urinary
tract anomaly.
