Hello! Is anyone using a consent form for domperidone? We have a compounding
pharmacist in town and doctors that were willing to write prescriptions
until an OB went on a Anti-Domperidone tirade. Now the docs would like to
have a consent form signed by the parents.

Anyone using a consent they would like to share? Is the FDA causing any
problems about domperidone or have they moved on to the next big "threat to
our health"?
