Speaking of nipple shields,what exactly are the guidelines for choosing the 
size of nipple shield? Sometimes when I choose the smaller (medela) size 
for a preemie the mother's nipple can't be drawn in the shield and there 
isn't good milk transfer. When I move to the larger size there is good milk 
transfer but the baby has difficulty getting or sustaining a good latch. Is 
there a good reference out there?
