I also found that having moms sit back in a chair, (instead of sitting
straight with a pillow behind them, and a pillow on their knees, and a
pillow under their arms, )

Makes a huge difference.  I am also trying to get moms away from pillows.
In the first 3 days, they all have a natural pillow, i.e. the remains of the
belly, and the baby can rest nicely on that.  By the time it goes down, they
won't need a pillow at all.  But the sitting back is the key.  Their whole
body relaxes and the weight of the baby is on them, and not on the pillow
with the baby pulling at the breast. The baby's body is tucked under her
other breast.  I use Rebecca Glover's flip the nipple, which Ayelet taught
me when she came to visit.  That way, you don't have the mom pushing the
breast toward the baby and getting a shallow latch.  IN most cases, they do
not continue holding the breast, but rather put their arm around the baby
and return to the cradle hold.
