It might work to do reverse pressure softening (RPS) - check our archives 
for details - and hand expression instead of pumping until the milk begins 
to flow. I would use cold cabbage leaves continuously until the milk began 
to flow. I would continue to use the ibuprofen around the clock. Feed the 
baby by alternate method. If baby can latch once the breasts soften a 
little, and if mom can tolerate the pain, I would use a supply line to give 
expressed milk (or donor milk or formula if needed) so that baby can suckle 
at the breast. Pain meds if needed and antibiotics if mastitis s/s not 
resolving with above measures.
I have heard rescue remedy homeopathic helps w/ stress, relaxing, and 
letdown.
