I have been helping a mum with her second baby who is having supply
issues. Day 10 now. Baby is very jaundiced, feeds for hours and hours
and never seems settled and still cries even after 4 hours on the
breast. Positioning and attachment are good but baby has a disorganized
suck for only brief moments before pausing. Mum has been switch nursing
and breast compression to increase intake. Also eliciting MER if
possible before bringing baby on to feed-he seemed to feed more
vigorously. No weight info on day 10 but day 5 was 2.6kgs, birthweight
2.8kgs. stooling only 2 times a day and not large stools. I helped this
mum to initiate breastfeeding during the first 3 days and all seemed
well-good sustained feeding and numerous stools, and yet now something
has happened during the week. The mum mentioned that she is staying with
her husband's family-she is feeling very oppressed and constrained by
their beliefs (they are from different cultures) and mentioned that for
the first 5 days after delivery they would not allow her to drink
anything-even water, because she was "too fat" and they "don't have fat
people in their family". The mum felt extremely hungry and thirsty in
the 5 days after delivery and was drinking barely 1 glass of water a
day. It is summertime here-48 degrees Celsius and homes are mostly
Airconditioned which is also very drying.  Do you think that this level
of dehydration in the first crucial 5 days could have such a negative
impact on the initiation of her production? After day 5 she has been
drinking more but it seems the undersupply, jaundice took hold and the
vicious cycle started and I suspect there was a delay to Lactogenesis 2-
is that possible with dehydration? Many thanks.
