Two things come to mind...

1)ductal yeast, which fits the description you offer.  It takes a longer
course of treatment, and scrupulous hygeine to resolve.

2) Ask mom if she has cold hands and feet.  Nipple vasospasm mimics
yeast symptoms, and can be treated with 30 mg Nifedipine daily until
symptomology is gone, or for the duration of breastfeeding, if need be
per Ruth Lawrence.

My gut instinct is to persue the ductal yeast first, as the mother has
already had a vaginal yeast infection from the antibiotic.
