I would think the leaking is occuring because these accessory sites lack
sphincter/other muscle tone? Usual treatment is to leave it alone or cold
compresses to encourage involution. If the sites are very near the nipple
then baby may be able to place mouth over. I would suggest to the mom that
w/ each pregnancy more breast development can occur and if she has OPTIMAL
bf practices from the start she should have a good chance of good supply.
All the prenatal signs sure are encouraging. Just monitor closely and follow
wts. This may be more of a confidence thing. As an illustration, I'll share
a case w/ you:

Had a client yesterday call becoz at 4 wks she feels her breasts are soft
and baby is nursing very frequently (to her). Pattern is normal to me and I
reassured her of this. Baby's always had infrequent stools. This is a mom
who had problems w/ poor bf and wt loss in first 2 wks and her pedi referred
her to us. We are followoing her about 2x/wk for wt checks and she recently
stopped supplementing ebm and is fully on breast. Wts have been good since
we "fixed the bf". So I had her come in for a wt check and the baby had
gained 9 oz in 5 days. It ain't broke, I've already fixed it! :)
