Roberta, that Mom really needs psychiatric help.  I have had kind of similar
experiences in the past.  Most of them start out as bottle feeders though,
because I think deep down they've already decided that they don't want the
baby to interfere too much in their lives.  I feel there is a tiny minority
of people who have kids b/c they think they're supposed to, it's what comes
next, after getting married and buying a house.  These kind of moms seem to
call me more often and ask me things like, but why isn't he sleeping all
night yet? and why does he want to be held all the time? and why isn't she
happy alone in her bed in her room?  I ask myself why did you have the kid
then?  It's a shame.  Anyway, I think you did the best you could with that
mom, and it may very well be best for her not to breastfeed.
Katherine, I agree with what you say.  Pediatricians telling moms to bottle
feed is kind of analogous (analagous?) to putting kids on antibiotics for
viral colds.  I teach this to residents and medical students all the time.
It takes less time to write a prescription for amoxicillin than to take the
time to teach parents that colds are viral and they have to run their
course, and that you're really sorry baby feels bad, but the best thing to
do is wait.  Baby will get better.  If mom is breastfeeding I have the
luxury of saying, your breastmilk is baby's medicine.  It also takes more
time to breastfeed counsel than to say give a bottle.  I really think that's
why a lot of peds do it.  Also they're to embarassed to admit they don't
know enough about breastfeeding.
