Melanie, thanks for the additional information.
Do you mean the child's length and head circumference are in the high percentiles?   If so, this is an unusually slender person, but one whose longitudinal and brain growth would seem uncompromised.  This does not have the hallmarks of failure to thrive.  It looks more like a case of an 'outlier', a baby whose measurements place her so far away from the typical child that the only way you can evaluate her health is to look closely at her, as herself and not in relation to all other children.  If she were listless and weak, or if her head circumference and length were also falling through percentiles, we might suspect she was not adequately nourished, but as it is it sounds like she is just light for her length.  
The father is long and lanky.  Sounds like she is his daughter.
I am not sure what the purpose would be of manipulating her diet and stopping breastfeeding, unless you consider that being light for one's length is pathological in itself, and can be cured by becoming plump.  
I'm glad she is going to find another doctor at the very least.
