I was giving an outreach lecture yesterday and a participant came and

asked me about evidence-based information regarding first water feeds

for bottle feeding babies.  I realize this is a breastfeeding listserv,

but thought maybe someone might have a classic article in a file

somewhere and be able to reference it!!   She is a newer nurse and had

done her nursing clinicals in our hospital and knew we didn't do that --

babies either go to breast or get formula right away, depending on mom's

feeding choice.  She indicated that their nursery in this smaller

hospital has a few nursery nurses who have been there "30 or 40 years",

and are very set in their ways....    They insist the bottle babies get

3 consecutive feedings of water before starting formula (helps them get

rid of mucus). 

 

Guidelines for Perinatal Care only state, "If the neonate is stable and

if BF has been elected by the mother, the neonate should be placed at

the breast in the delivery room within the first hour after delivery."

Several sources talk about not using water supplementation for

hyperbilirubinemia, etc., but frankly I think the practice of initial

water feeds is such an ancient one, that nobody even talks about it in

books anymore......    

 

I did suggest to her that she bring it up again, perhaps to the DON or

someone.  Maybe they need to bring their documentation in support of the

practice (the hazard with asking for that is the apparent difficulty in

finding an article to dispel the myth of needing to do initial water

feeds in order to support discontinuation of that practice!).

 

So, if anyone knows of an article or reference in a book to state you

shouldn't do water feeds first in the nursery, I'd appreciate the

reference/help.

 

If from a book, you could fax the title page of the book along with the

appropriate pages if able.
