Placenta fragments can and are left behind from a c-section.  About a month
ago, there was a discussion here about it, check the archives.  On the other
hand, I have seen lactogensis II take an extremely (2+weeks) long time to
come.  In one woman with around the clock pumping, baby at the breast, etc.
etc. she did not get a full, life sustaining milk supply until 8 weeks.  She
would say, "If only he would breastfeed well, if only I had enough milk,"
Now she is calling asking about how to get her 14 month old off the breast
every 2 hours.  Be careful what you wish for!!!  Be patient, be diligent.  I
am so proud of this mom, she really hung in there and kept going when she
thought she couldn't.  There was no apparent reason for her slow milk
development except at one point she said to me,  "You know, I have always
been a late bloomer, I didn't even get my period until I was 16."???
