Well no ideas just a "hmmm" from me.  I am a bit behind in my reading but
here goes.  I just got the same compliant from a mother with a 3 day old
baby.  The baby prefers her left breast and her left nipple is also
smaller.  I wonder if she may an accessory nipple also.

If I may vent for a while about this call.  Part of the problem was the
baby wasn't taking the breast and was sleeping alot.  I asked her how often
does the baby nurse, answer I wake the baby every 3 - 4 hrs and she nurses
for about 5 minutes.  That is it, how many wet and dirty diapers?  One
dirty one every day , but not wet diapers in the last 2 days (the baby is
only 3 days olds).  She had been pumping to help her breasts even out.  I
tried to not scream at her feed the baby.  I told her she had to feed the
baby the milk that she was expressing (I didn't care how, spoon, cup,
BOTTLE) and to wake the baby every 2 hrs (3 at night).  I told her after
each feeding (maybe an hour later) to give the baby some expressed milk and
that she should get a wet diaper by evening (she called in the am).  UGH! 
Worrying about nipple confusion is a good thing, but not to avoid feeding
the baby.  That is why she hadn't given the baby a bottle, to avoid nipple
confusion.  I told her about 3 times to feed the baby any way she could I
just hope she listened.
