This wonderful first time mom I have worked with has ongoing problems that
just never seem to resolve. Baby is now over two months old. Mother had
chronic plugged ducts from beginning and treated them appropriately with baby
getting better latch, pumping with Symphony, using Lecthin, compresses,
homeopathy, etc. She has a history of fibrocystic breasts but these plugged ducts
tended to be in a line and were relieved upon nursing or pumping, but came
back. She is still taking Procardia for high blood pressure that she had during
pregnancy. Baby transfers milk well and has had appropriate weight gains,
output, etc. Just when things were improving and breastfeeding was settling
in, both developed Thrush. Mother had forgotten about the antibiotics
administered during labor. Now, baby has been on Nyastatin and mother on Diflucan
for about a week or so. Doctor did not want to prescribe APNO, because she
said that there was nothing "red or itchy" on breasts. Mother called the other
day in a panic because her plugged ducts were so much worse and came to the
conclusion that perhaps, this had been related to Thrush all along and once
that would be resolved with Diflucan, so would the plugged ducts. I wondered,
but hoped. She has just called and now has about 4-5 milk blisters or
blebs, she thinks and wants to aspirate them to relieve the pain, relieve the
worsening plugged ducts and get her milk flowing. She says that olive oil feels
good, but she does not want to use the Nyastatin cream the doc gave her to
apply to her nipples any longer, b/c she thinks that has caused all these blebs.
Of course, to add to the mess -- she is returning to her demanding job as
an attorney in the next couple of weeks! Your thoughts??? Help! Thanks, Ann
