Hi Catherine,
I was just reading a new article about nipple eczema last night which 
mentions Paget's disease of the nipple, but says it has not been reported 
(yet) in a breastfeeding woman (Barankin B, Gross MS. Nipple and areolar 
eczema in the breastfeeding woman. J Cutan Med Surg. 2004 
Mar-Apr;8(2):126-30.)
I also mentioned this in the case study of nipple eczema that I wrote in 
1993 in JHL, but have not heard of it occurring in a breastfeeding woman.
If the surgeon is not interested in taking a biopsy from the areola, perhaps 
she could find a dermatologist who would do it.
However, it does sound like a large mass if it is 4-5cm, so a mastectomy may 
be necessary.
