Hi All,
I have a group mom who called me in a panic this morning.  First baby,
three months old, thriving breastfed happy couple.  Baby slept longer than
usual last night, woke in early am, nursed well on left side, fell asleep
so barely nursed on right side, both baby and mom slept a couple hours
longer, then mom awoke to find her right breast firm, pink, and engorged.
She knew to do all the "right things", tried warm soaks, nursing and
pumping on that side (she is an experienced pumper), but says she is unable
to get a letdown and as of 12 noon has not been able to drain any milk from
the right side.  Baby will latch on right but gets frustrated because "he
is not getting anything", nursing normally on the left side.
My suggestions to mom included continuing with warm soaks/compresses,
manually expressing a little around the areola to soften and allow a better
latch, frequent attempts to nurse on that side, relaxation/nursing laying
down or in changing positions, small dose of ibuprofen for inflammation,
and apply chilled cabbage leaves.  Also gave her signs of mastitis to watch
for.  What other suggestions would you make?
