I cannot speak for Betsy, but I have seen moms "for whom (breastfeeding)
would not (and should not) ever be the choice:
Women actively abusing drugs and alcohol
Women too self-possessed to recognize their baby's needs
Women mentally challenged and unable comprehend the baby's needs
Young girls pregnant as a result of physical abuse/rape

Any of these women, given an extremely supportive environment, may be able
to successfully breastfeed a healthy and happy baby.  But, many of them have
little support, and have ecomonic circumstances that make life especially
challenging.  With bottle feeding, at least someone else can step-up and
provide food for the baby so that it won't starve.
