How frustrating for this mom! Did she bleed heavily postpartum? Does
she have postpartum thyroiditis? I read recently that this either
worsens or resolves by about 3 months postpartum. (Is that correct in
your experience?)  I liked Barbara Wilson-Clay's "premie plan": nurse
then pump and feed the pumped hindmilk as a supplement. Maybe it
should be fed by cup or SNS or Lact-aid. I assume the mom is not
giving pacifiers. You wrote that she is pumping frequently. Is she
using a hospital-grade electric double pump and is she
"double-pumping"? I have learned to my surprise that many people don't
understand the differences between pumps and that many do not
understand the value of pumping both breasts at once. More than once
have I heard "Yes, I have a double pump. No, I just pump one side at a
time and boy! does it take forever!"(stuffing lunch with the other
hand!). (=O?????!!!!!)  You didn't mention herbal supplements. Has she
considered these? The best I can tell, fenugreek, blessed thistle,
fennel and hops(fresh) are reliable as galactogogues. From what I
read, tinctures of fenugreek and blessed thistle work within 24-48
hours. What about Mother's Milk Tea? I have no experience with it,
anyone out there tried it? Here again is good time to find another
HCP, since hers doesn't understand the value of helping her work
through this problem instead of dismissing it with, "Better luck next
time!".  Good luck.
