Another scenario that I have seen is: baby has a tongue restriction (even a
subtle one), lives on MERs for the first month or so and then when it's
completely up to baby to drive supply he can't, mom's supply declines and
baby begins to lose wt and get frustrated at breast due to low supply and
his inability to remove milk well.  Often bottlefeeding is a relief to
everyone b/c baby can more competently feed this way.
