Estradiol contains estrogens which are known to decrease milk production. As
it is in the form of a cream and is applied to the mucous membranes of the
vagina, it will pass into the woman's body but I have no idea at what
percentage it is absorbed - this cream is to treat vaginal dryness so needs
to treat locally only although it will surely pass into the blood. Ideally
this woman's milk supply would be very well established and she'd be passed
her first few months of lactation before she uses this medication. If she
has any concerns about her milk supply, she should not be using this. Also,
there is concern as to the increase hormones the baby will be getting from
breastmilk. In LactMed, the mobile version, all estrogen containing meds say
the same thing:  it says that " vaginal administration results in measurable
amounts in milk but patches do not. Maternal doses of up to 100 mgr daily
transdermally produce low levels in milk..Vaginal administration results in
unpredictable peak times for estradiol in breastmilk... A case report of
inadequate milk production and inadequate infant weight gain was possibly
caused by transdermal estradiol initiated on the first day postpartum but 2
small studies found no such effect when the drug was initiated after
lactation was well established."
