I have permission from a client to post. She has a 5 day old baby who was
born by C-section at 37 weeks. Her birth weight was 6 lb. 13 oz. Do not know
present weight. This baby fights breastfeeding and when she finally does get
her to latch she is off within the first five minutes. But on some occasions
stays on longer. She starts choking within the first five minutes and comes
off the breast. When she tries to get her going again she roots but does not
want to get back on again.
She is gassy. This all seems to be worse at night. She said she seems to be
hungry too soon after any nursing session. From what I can find out this all
seems to point to an overactive let down reflex. Possibly oversupply also.
From what I can find in the archives I should first recommend the "up hill
nursing positions." This is where the Mom leans back in a recliner or baby is
propped up at a level above the moms nipple and rolled over somewhat above
mom's chest. The side lying position with baby nursing with the breast
closest to the bed is helpful also. Other than this possibly the mother
should express some milk first to over ride this too sudden of let down. If
none of this works then suggest to her to possibly get some nipple shields to
slow down the flow. Then also suggest possibly to feed on only one breast
every feed. I assume so there will be no foremilk / hindmilk imbalance.
Should I ask her the color of the babies stools now to see if this is an
issue now? I think I have thought of everything but any suggestions would be
greatly appreciated.
