   I just wanted to provide a personal experience to help you a bit. I
delivered a 24 wk baby 12+ years ago. Because of several things
(including how long ago it occurred) I was not even questioned or
encouraged to consider breastfeeding. At the time, the HCP were just
trying to keep him alive. I did not have any sensations or leaking of
colostrum or milk until three days postpartum (this was #3, I had
breastfeed both previous children). By day 3 the baby had died of
complications unexpected and possibly unrelated to the prematurity.
  My personal recommendation is to encourage the mother to continue
pumping and to remind her that her babies are very premature and it will
take her body a while to get working correctly. It is not her 'fault' or
the hospital's 'fault,' but rather just a by-product of her unique
situation. I would also recommend Helen Harrison's book - The Premature
Baby - it is an excellent work on all aspects of premature parenting -
including breastfeeding and with a better understanding of her situation
and her babies, the stress may be reduced a bit.
