A new mom in my practice was diagnosed with Sjogren's syndrome 2 years
ago.  She took pilocarpine until she got pregnant with her now 1 week
old baby.  Breastfeeding was initially challenging, mostly due to the
baby's recessed mandible and small tongue.  We got him latched
asymmetrically, and her pumping to relieve her engorgement, and all
seems to be proceding normally, with the baby gaining an ounce a day,
after a greater than usual initial loss.

She and her husband are concerned about the possibility of breastfeeding
being affected by the Sjogren's, since it causes dysfunction of
secretory glands.  Lawrence has one paragraph stating that Sjogren's can
cause lactation failure.  A medline search revealed nothing.  I am
interested to know if anyone has any experiences with this disorder in
their practice...

She is starting to become symptomatic again, and would also like to know
if there is any updated info on the safety or milk transfer of
pilocarpine since Hale's redbook 1999 was published.
