"I'm looking for the original WHO reference that states the preferred
order in which a baby should be fed" (breastfeeding, mother's expressed, donor, commercial formula)

The reference I use is "Facts For Life", put out originally by UNICEF, WHO, and UNESCO in 1989.  My edition, 2002, is put out by UNICEF, WHO, UNESCO, UNFPA, UNDP, UNAIDS, WFP, and the World Bank ("over 15 million copies in use worldwide").  It's a lovely, succinct (153 pp) little book that can be ordered or downloaded from http://www.unicef.org/ffl/.  The hierarchy is on page 48.
